Please note: This medication refill request form is exclusively for current clients whose pets have had a recent wellness exam. If you are a new client, kindly contact us at (703) 779-2903 to schedule a wellness exam and establish care for your pet. A wellness exam by one of our veterinarians at Old Mill Veterinary Hospital is required to approve any prescription refill. Additionally, certain controlled substances may require a more recent exam before we can fulfill your request.
